New Client Information

Prior to your first session, I need to collect your health information for my records. Please choose one of the following three options below.

1. Access and print the forms below to complete at your leisure, and bring them with you to the session.

2. Fill out the confidential PDF client health intake form and email it back to me when it is complete.

3. Arrive at the treatment space 10 to 15 minutes early and complete the forms prior to the start of your session.
